DUPLICATE CHECKS/WARRANTS

Policy 3.9
Kiamichi Technology Center ("KTC") may issue a second or duplicate check or warrant in lieu of any check or warrant issued and subsequently lost or destroyed by the payee. No second or duplicate check or warrant will be issued until KTC has stopped payment on the check or warrant by the payor bank or, in the alternative, until an affidavit explaining the circumstances regarding how the original check or warrant was lost or destroyed has been submitted to KTC by the payee. The appropriate administrator may use his/her discretion in determining which alternative to use to preclude any KTC loss, taking into account KTC's past relationship with the payee, the amount of the original check, and other relevant factors.
